This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free. Primary Care Health Services, Inc. provides all phases of general dental health services for infants, teens and adults. Treatments Include: Pediatric and Adult Dental Services. Examinations Cleaning X-rays Fillings-White ("bonding") and silver Dentures (Complete & Partials Dentures) Emergency Tooth Care Pediatrics (Cleaning, Exams, Fluoride and X-rays) Head Start Exams Fluoride (for children 16 and under) Sealants Extractions Root Canals-(for Incisors and Bicuspids) Gum Tissue Care Veneers Crowns Bridges The Sliding Fee Program offers a reduction or discount on all health care services except prescription drugs and is based on household income and family size. Patients may qualify to only pay 25, 50, or 75 percent of the cost for their health care services. Dental Hours: Monday, 11:00 am - 7:00 pm. Wednesday, 9:00 am - 5:00 pm.
