This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free. Provides comprehensive primary health care including screenings and immunizations. Provides basic dental care. Hours: Monday, Tuesday, Wednesday,Thursday 7:30 AM - 6 PM Friday 7:30 AM - 11:30 AM (walk in only) Appointment preferred; Walk-ins accepte Fees: Medicare, Soonercare, Medicaid Private Insurance accepted. sliding fee scale Must provide proof of income for sliding fee scale; shot records for immunizations; photo ID.
