This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free. Services Include: Comprehensive Exam Dental Cleaning Fillings Partials Crowns Root Canals Dentures Gum disease treatment Extractions Orthodontic referrals Specialists referrals when needed Offers a Sliding Scale Fee Discount Program. CommWell Health seeks to make its services more affordable for anyone who qualifies based on income and family size. Typically the discount program benefits individuals who are uninsured, have high deductibles, or have low income. This program provides a discount off of the health center’s normal charges and applies to most, but not all, of the heath center’s services. Work Hour Monday, Tuesday, Wednesday: 8:00 am – 5:00 pm Thursday: 8:00 am – 8:00 pm Friday: 8:00 am – 1:00 pm
