This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free. First Nations serves everyone.  As a Title V Urban Indian Health Center, First Nations also offers services at no cost  to eligible Native Americans.  All American Indians with a Certificate of Indian Blood (CIB), of all tribal affiliations, are eligible to receive the following services at no cost. NO PATIENT will be refused service/treatment based on their inability to pay. You will pay a minimal co-payment for each visit and/or medication Patients may apply for a discount rate if they have no other source of payment. Income and number of household members determine discount amount. First Nations Community HealthSource offers a comprehensive dental program which includes the following dental services:     Exams and X-rays     Prosthetic services (bridges, partials and dentures)     Emergency and pain management     Cleanings     Endodontic Services (root canals)     Restorative (fillings and crowns)     Oral surgery services (extractions) Work Hour   Monday – Friday: 8 a.m.-7 p.m. Saturday: 8 a.m.-12 p.m. Sunday: Closed
