This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free. Dental Dental Cleaning Dental Restorations (fillings) Dental X-RayAlso offers an array of other services ranging from x-rays to lead testing. For a complete list, please visit their website. Provides a sliding fee discount program to those who qualify. To qualify for the sliding discount, you must fill out a form (these can be picked up at any of the offices) and bring proof of income into any of our offices. A minimum payment of $30 per Medical visit and 50% per Dental visit, is expected at time of services. While they are not a free clinic, they are able to provide reduced costs to those who qualify while providing quality health care to all. Work Hour Monday - Thursday: 8 am - 12 pm & 12:30 pm - 5:30 pm Friday: 8 am - 12 pm Saturday: Closed Sunday: Closed
