This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free. Provides access to life-changing, essential orthodontic treatment for qualified children from low-income families. Children ages 10-18 with a moderate to severe crooked teeth or jaws, good oral hygiene, no more than 4 baby teeth and not already wearing braces.Hours:Limited phone hours Tuesday-Friday / 1-3 pm Central Time; voicemail messages left outside of the designated phone hours will not be returned; email is the best and fastest way to reach agencyFees:$30 application fee and $650 program fee if approved for treatment through SCL.Payment options: Low-Cost/Sliding Scale
