Park DuValle Community Health Center is committed to your health care and endeavors to provide the highest quality, culturally sensitive, prompt and effective care to each and every patient regardless of race, religion, creed, ethnic origin, disability, age or economic or social status. Services include: Dental Care. Hours: Monday, Wednesday, Thursday and Friday 8 AM - 5 PM Tuesday 8 AM - 8 PM Saturday 9 AM - 4 PM As a federally qualified health center, all of PDCHC's sites see patients regardless of ability to pay. Low-income and uninsured patients must apply and provide documents to prove eligibility for discounted health services as determined by Federal Poverty Guidelines and must be reverified on an annual basis. Patients with KY Medicaid, Passport, Medicare or Private Insurance are also welcomed. Contact to find out more about discount service eligibility or insurances that they accept.
