This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free. The Jane Pauley Community Dental Centers offer a range of dental services, including cleanings, fillings, sealants, and routine extractions. Patients requiring other dental procedures, such as root canals, dentures, crowns, and implants are offered referrals to a patterning organization who provides these services at affordable rates.Our dental centers see both children and adults, offering a child-friendly atmosphere and approach. Parents wishing to accompany their children to the exam or procedure are always welcome. The staff at our dental centers are professional and friendly, helping to put patients at ease. We at Jane Pauley understand that dentist visits can be stressful for some, and we help to alleviate that stress and make the visit as enjoyable as possible.Payment options: JPCHC Sliding-Fee-Scale: Jane Pauley Community Health Center’s internal discount program for patients with qualifying income. Hoosier Healthwise (HHW): Indiana’s health care program for children 0-18 years of age, offering medical, dental, and vision coverage at little or no cost. Healthy Indiana Plan (HIP): Indiana’s health care program for uninsured adults, offering medical, dental, vision, and pregnancy coverage for a minimal, monthly contribution based on income and family size. Presumptive Eligibility (PE): A temporary coverage for services only while your eligibility is being determined for other Indiana coverage. JPCHC Prescription Assistance Program: Our health center collaborates with various pharmaceutical companies to provide select medications to uninsured patients free of charge based on household size and income. Health Insurance Marketplace: The program where individuals and small businesses not eligible for Indiana’s plans can shop for and buy private health insurance at reduced costs based on income and family size. Most applicants qualify for premium tax credits that lower the costs of coverage. All plans cover essential health benefits, pre-existing conditions, and preventive care.
