This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free. Office hours: Mon, Tue, Wed, Fri: 7:00am-5:00pm Thur: 8:00am-6:00pm (closed every other Wednesday or Friday) Grand Peaks Medical and Dental is a non-profit Community Health Center dedicated to providing quality primary and preventative healthcare as well as health related services to all members of the community. Grand Peaks is a Federally Qualified Health Center (FQHC) that began as a small medical center in 2007, with one Provider and a small staff. Serve patients in St. Anthony, Rexburg and surrounding areas of the Upper Valley. As a non-profit Community Health Center, the Grand Peaks organization is committed to providing affordable, quality medical and dental care to patients. Whether you are insured, self-pay, uninsured, or low-income, offer affordable rates. These rates are discounted on a sliding fee scale based on proof of patient/family income and household size.
