Service programs provide general dental services for older adults ages 50 and older within the 300% Federal Poverty Guidelines.Documents required:Photo ID, proof of income, and proof of residency for Collier CountyHours of operation Monday – Thursday from 8a-4pm.Accepted insurance: Medicare and Medicaid. Sliding scale is available base on household income.
