About This Clinic

This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free. The mission of the Summit Community Care Clinic is to provide primary and preventative health care to low income, uninsured, and under-insured people who live or work in Summit County at a cost they can afford.Hours: Monday, Tuesday and Friday: 8:00 am - 6:00 pm.Wednesday and Thursday: 9:00 am - 6:00 pm.Dental Services: Fillings Extractions Periodontal cleanings Child and Adult Cleanings Dentures Root Canals Crowns Emergency appointments BridgesDuring the eligibility process, our eligibility technician will review your income and place you on our sliding fee scale accordingly. Fees are accrued based on the care you receive, and so it is difficult for us to know how much your charges will be before your appointment. We can only give you estimates of costs. We ask that you pay your entire bill at the end of each visit, however we are willing to work with patients on payment plans. We want our patients to receive the care they need and maintain health regardless of their ability to pay at the time of their visit.
